Please feel free to download our Patient Medical History Form to present on the day of your appointment. Alternatively, you can fill it out in our waiting room. Please arrive 10-15 minutes before your appointment to allow time. We look forward to meeting you and showing you The Happy Tooth difference.

Our dental practice is located on the ground level of 126 John Street. There is street parking on John Street as well as a large 2P free public car park at the back of the arcade which is accessed via Burns Lane.
